This privacy policy explains how MedTrans Express Inc. (“MTX”, “we”, “us”) collects and uses information through our website, medtransexpress.com, and when you contact us.

Information you give us

When you request a ride on our website, we ask for:

• Your full name and phone number
• Your email address (optional)
• Pickup date, pickup time, pickup address and destination
• Whether you need a wheelchair accessible vehicle, and whether you need a return ride
• Any notes you choose to add

When you apply for a job on our website, we ask for your name, phone number and email address, and anything you choose to tell us about yourself.

When you call, text or email us, we receive the information you choose to share.

How we use your information

We use your information to respond to your request, arrange and confirm your transportation, and answer your questions.

Job applications

Applications sent through our careers page go to our hiring team and company management. We use them only to review your application and contact you about it.

How your information reaches us

The ride request and careers forms on our website open your own email app with your details filled in. Nothing is sent until you press Send, and your email provider delivers the message. Our website does not store the details you enter.

Keeping your information safe

Our website uses encrypted connections (https). We protect members’ health information as required by HIPAA.

Because the ride request and careers forms open your own email app, please don’t include medical details in them. For a medical emergency, call 911.

How long we keep records

We keep member records for 7 years after a member’s profile becomes inactive, as required by AHCCCS and other state and federal record keeping and audit rules. For members who still use our service, we keep their records for as long as their profile is active.

Sharing your information

We share rider information only as needed to arrange and pay for your ride, for example with your health plan or AHCCCS, and when the law requires it. We don’t sell your information.

Text messaging

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Cookies and other services

Our website does not set its own cookies, does not use analytics or advertising tracking, and does not take payments. Some content on our pages comes from other companies:

• Google Fonts: your browser loads our fonts from Google, which receives your IP address.
• Google Maps: the map on our home page is provided by Google, which may receive your IP address and set cookies when the map loads.
• Our website host: keeps standard server logs, such as IP addresses, to run and protect the website.

Your choices

You can ask us to see or correct the information you have sent us, or ask us to delete it. We may need to keep some records for the time described above.

Changes to this policy

If we change this policy, we will post the updated version on this page with a new date.
